INSTRUCTIONS AND GUIDELINES FOR SOLICITING LOCATION PREFERENCE

FOR THE POSITIONS OF BLOCK PROGRAM MANAGER, BLOCK ACCOUNTS MANAGER AND DISTRICT HOSPITAL ACCOUNTANT UNDER NATIONAL HEALTH MISSION, MADHYA PRADESH

An online application has been developed for soliciting Location preferences from the candidates shortlisted for positions of Block Program Manager, Block Accounts Manager and District Hospital Accountant under National Health Mission, Madhya Pradesh. Candidates can provide his/her location preferences through the online application form.

Following are the steps, processes, and instructions for providing the location preferences.

(1) Candidates need to login on Online Application from the below provided link:
BPM BAM_DHA/ with their Roll Number and DOBS
(2) Candidates need to choose the locations from the dropdown box to provide their preferences for the allotment.

(3) Candidate must provide the preference for all the available locations. In case the candidates don't provide location preferences for all the available locations within the stipulated time, their candidature shall be considered for all the locations and for such candidates, a random location shall be allocated to them;

Note: Candidates shall not be able to submit their location preferences form without providing preferences for all the available locations

(4) Signing of the self-declaration: "I fully understand that details of location preference, as above, are being solicited for information purposes only. Job postings shall be decided on the overall merit rank and location preferences of the selected candidate. I fully agree to abide by the location of posting determined by NHM, MP through the above process
or as per their discretion and availability of the locations." 
(5) Preview of selected location and preferences before final submission;
(6) Final submission of location preference shall be based on One Time Password (OTP), OTP shall be sent on Registered Mobile Number (RMN), the candidate shall submit the form after inputting the right password (OTP) sent on RMN;
(7) Four (4) days of the time shall be given for submission of the location preferences starting from October 20, 2021, 4:00 pm till October 24, 2021, 3:59 PM for the above positions which written test was held on August 21, 2021.
Note:
(a) Candidates shall be able to modify the Location Preferences till the last date of submission. In case of extension on the last date, previous submitted location preferences details cannot be modified. (b) Candidates who fail to provide location preference within the above stipulated period or during any extension period, for such candidates' 'random location' allotment shall be done.
